In this follow-up, we're diving deeper with 10 additional questions to include on your tattoo consent and booking forms. These will help you cover key topics like health conditions, consent for photos, and aftercare awareness - all the little things that make a big difference. Additional Questions to Make Your Booking Form Even Stronger

1. Skin Condition & Sensitivities

Ask clients whether they have any skin conditions (eczema, psoriasis, keloid-scarring) or known sensitivities (to inks, latex, adhesives). This helps you plan the session safely and advise on any precautions.

2. Recent Sun/Tanning or Peel Treatments

Include a question like: “Have you had a sun-bed session, spray-tan, chemical/laser peel in the last 4 weeks?” Skin recently treated can affect how the tattoo heals, and it's good to know beforehand.

3. Are You Taking Any Blood-Thinning Medications or Supplements?

Having a field to ask about blood-thinners (both prescription and over‐the‐counter supplements) helps you anticipate extra bleeding, longer healing times, or the need for added aftercare instructions.

4. Pain Tolerance & Relaxation Needs

Some clients may be more nervous or sensitive to pain. A question such as “On a scale of 1-10, how would you rate your pain tolerance?” or “Would you like periodic breaks during your session?” makes the guest feel heard, and you can plan accordingly for comfort.

5. Tattoo Aftercare Plan

Ask: “Are you comfortable applying after-care as instructed (keeping it clean, avoiding sun/swimming)?” This isn't just about compliance-it gives you a chance to flag key after-care cues in advance and highlight what you expect from the client to get the best result.

6. Reference Image License/Ownership

If a client is sending you a photo or design from someone else (another artist, copyrighted art), include: “Do you have permission to use the reference image/are you aware of copyright/influences?” This helps protect you (and them) from legal or moral issues.

7. Consent for Photo/Progress Sharing

Many artists like to share before/after or in-progress shots on their social media or website. Add a question: “Are you okay with your tattoo (and possibly your image) being photographed for social media/portfolio use?” This clarifies consent upfront and avoids surprises.

8. Cancellation & Rescheduling Awareness

Formally ask: “Do you understand the studio policy for cancellations and rescheduling (e.g., deposit forfeiture, minimum notice)?” While you may have this in a separate policy document, including it on the form ensures the client acknowledges it as part of the booking.

9. Travel & Accommodation (for out‐of‐town clients)

If you sometimes book clients from out of town, ask: “Will you be travelling and/or staying overnight, or do you have other timing constraints (flight, work schedule)?” This helps with planning multi-day sessions or coordinating timing with hotels/flights.

10. Emergency Contact

Though not always required, asking for an emergency contact (name & phone) can be wise, especially if you're doing a long session or working on a large piece where relief breaks or unexpected issues might arise.
